Lumary AC - This article explains how to generate a care plan for a goal achievement plan.
Once you've finished setting up your client's goals, the next step is to generate a care plan to provide to the client. The care plan documents a client's needs, goals, preferences and how funded aged care services will help the client's realise their goals.
To generate a care plan:
- Navigate to your client's contact record.
- Click on the Goals tab.
- Your client's care plans will be listed in the Goal Achievement Plans section with a record type of Care Plan. If you can't see the plan you're looking for, you may need to click on the View All text link.
- To open a plan, click on the text link of the relevant plan name.
- Click on the Generate Care Plan PDF button.
- Depending on how your environment is set up, this will launch a modal that will track the progress of the document generation. Once this has finished, the modal will automatically close.
- To open the document, click on the Related tab.
- Scroll down to the Files section.
- Click on the file that contains the Care Plan.
