Lumary AC - This article explains how to create a care plan via the goal achievement plan.
By setting and monitoring goals, you can show how your support and services help clients improve their health, wellbeing, and quality of life. You can create care plans and track your client's progress to achieve the goals outlined in their support plan or as agreed in their care plan.
You can also create a care plan goal achievement plan from a client's onboarding process. This may depend on your business's onboarding processes.
To create a care plan goal achievement plan:
- Navigate to the client's record.
- Click on the Goals tab.
- Under the Goal Achievement Plans list.
- Click on the New button.
- Select the Care Plan record type
- This will launch the New Goal Achievement Plan: Care Plan form. The following table describes the fields on this form.

| * Plan Name | An identifiable title for the client’s plan, helping to easily reference and distinguish it from other plans. |
| * Client | The system will auto-populate this field with the client's name. |
| Related Service Agreement | Current approved Service Agreement for related client. |
| Assigned To | The person who developed the plan. |
| Description | A description or any comments that will help describe the plan. |
| Alert Summary | Text to display as alert |
| Authorised By | The name of the person who authorised the plan. |
| Consenting Person | The name of the person who has given consent. |
| Relationship of Consenting Person | The relationship of the person who has given consent. |
| Report Due Date | The date by which the next care plan review is due. |
| Display As Alert | When checked this Goal Achievement Plan will be displayed as an alert on the Client's record. |
- Once you've filled in all the fields on the form, click on the Save button.
This will take you to the new Goal Achievement Plan record. You can now start adding goals to the care plan.
