Lumary AC - This article explains how to add a new goal and outcome to a client's existing care plan.
When a new goal is identified for a client with a care plan, you can add it to the client's goal achievement plan.
Once you've created a goal achievement plan, you can capture goals from the client's support plan.
To add a new goal:
- Navigate to your client's contact record.
- Click on the Goals tab.
- Your client's care plans will be listed in the Goal Achievement Plans section with a record type of Care Plan. If you can't see the plan you're looking for, you may need to click on the View All text link.
- To open a plan, click on the text link of the relevant plan name.
- Scroll down to the Goals section
- Click on the New button.
- This will open a New Goal form. The following table describes the fields on this form.

| Goal & Outcome Name | Mandatory. Clear Name for the goal, making it easy to identify and track progress against specific client outcomes. |
| Plan | Mandatory. The system will auto-populate this field with the client's plan. |
| Goal Detail | Mandatory. The goal as described on the client's Support Plan. |
| Outcome Detail | Mandatory. The outcome as described on the client's Support Plan. |
| Goal Summary | A concise description of the client’s goal, outlining what they want to achieve and giving context for support planning. |
| Reason | Reason for not addressing a goal |
| Obstacles | Any client's areas of difficulty or activities where the client needs support in order to achieve this goal as described on the client's Support Plan. |
| Goal Type | Identifies the timeframe or focus of the goal e.g. Short Term, Long Term, or Wellbeing |
| Goal Achieved | When checked, identifies that the client has achieved their goal. You'll usually skip this field when you're setting up a new goal. |
| Achievement Comments | Record reflections or notes on how the client has achieved the goal. Achievement Comments is required when the Goal has been Achieved. |
| Goal Archived | Checkbox that you can use later to archive a goal. |
| Archive Reason | Records the reason a goal is being archived, selected from the drop-down list. Archive Reason is required when the Goal has been Archived. |
| Other Archive Reason | Captures an alternative reason for archiving the goal. |
- Once you've added information about the client's goals, you can list the steps that the client will need to take to achieve them. For each step, you can include a due date and an indication of who is required to complete each step.
- When you've added all of the steps required to achieve the goal and outcome, click on the Submit button.
Your client's goals will now appear in the Goals section. You can click on the headings to reveal more information about each goal. Once you've added new goals, you can add the services that will help the client meet these goals.
