Lumary AC - This article explains how to track the progress and achievement of goals against a goal achievement plan.
Once you've added some goals and outcomes to your client's plan, you can track and monitor their progress. The system helps you to:
To get to a client's goal achievement plan:
- Navigate to the client's contact record under the Contacts tab.
- Click on the Goals tab. You might need to use the More drop-down list at the end of the row of tabs.
- Scroll down to the Goal Achievement Plans section.
- Click on the name of the plan.
- This will open the goal achievement plan record. If it's not already open, click on the Goals and Outcomes tab.
Add progress
To add progress to the goal achievement plan record:
- Click on the Related tab.
- Scroll down to the Goals and Outcomes section and locate the record you need to update.
- Click on the drop-down arrow at the end of the row and select Edit.
- Scroll down to the Outcome Tracking section. You can set the client's current outcome level by clicking on the Current Outcome drop-down list and updating the number.
- Then scroll down to the step that the client has completed. Click in the Date Completed field and set the date that the client completed this step.
- Once you've added dates to all of the completed steps, click on the Save button.
Show a goal achievement
As your client achieves their goals, you can set them to achieved. To finalise the achievement of a goal:
- Navigate to the goal achievement plan using the steps described above.
- Scroll down to the Goals section and locate the record you need to update.
- Click on the Edit button.
- Make any progress updates using the steps provided above. You may need to update the current outcome level and completion dates of any outstanding steps before setting the goal to achieved.
- Scroll to the Goal Achievement Information section.
- Put a tick in the Goal Achieved checkbox.
- Add any notes that will support the achievement to the Achievement Comments free text field.
- Click on the Save button.
- When you view the goal you'll see that it now has a status of Archived.
- The goal will now appear with a status of Achieved.
Archive a goal
You may decide for some reason that a client's goal is no longer relevant or not achievable. In that case, you can't delete the goal but you have the option of marking it as archived so that a record of the goal is retained for future reference. To archive a goal:
As your client achieves their goals, you can set them to achieved. To finalise the achievement of a goal:
- Navigate to the goal achievement plan using the steps described above.
- Scroll down to the Goals section and click on the Edit button.
- This will open the Edit Goal form. Scroll down until you find the Goal Archive Information section.
- Put a tick in the Goal Archived checkbox.
- Click in the Archive Reason field, and select a reason from the drop-down menu. If required, add a reason to the Other Archive Reason field.
- Click the Submit button.
- When you view the goal you'll see that it now has a status of Archived.
